Juanethea Bryant-Lord, NBC-HWC, is the Chief Executive Officer of 360 Chronic Care Management, operating as Primary Care 360, and a National Board Certified Health and Wellness Coach with more than 25 years of leadership experience in healthcare administration, chronic care management, and business operations. She is a graduate of Ashworth College, holding a Career Diploma in Health Science (Nutrition), credentials in Bookkeeping, with additional education in Business Management and Accounting, and is Master Certified in Health and Wellness through the Dr. Sears Wellness Institute. Juanethea has held senior leadership roles with Corners of Care LLC and Genesis & Associates Development Group, serving in executive and managerial capacities overseeing operations, finance, staffing, and Medicaid-based case management. Her leadership integrates operational excellence, fiscal accountability, and whole-person wellness to advance equitable, sustainable healthcare outcomes.
With a love for people spiritually first and foremost, she uses her expertise to provide resources, tools, and education for better outcomes in their journey towards health. In today’s society, we often see a gap in the healthcare system where certain individuals may fall through the cracks. They may not be elderly or severely disabled, but they are still struggling with health issues that greatly impact their quality of life. Recognizing this need for expanded services, two years ago, she embarked on a new journey to establish 360 Chronic Care Management. This company aims to provide resources, education, and insight to help better manage and potentially even reverse certain diagnoses. As a strong believer in Jesus Christ and His gospel, she is driven by her love for Him and her desire to make a positive impact in the lives of others. She also values spending time with her family and friends, which further motivates her to bring holistic options to those in need of care. With 360 Chronic Care Management, she hopes to bridge the gap in healthcare and help individuals live healthier and happier lives.
